Active and Continuous Practice: With respect to all legal employment in which you have engaged, subsequent to your licensure in each such country or U.S. jurisdiction and during the last seven years, please provide brief but complete information regarding the nature, substance, extent (approximate number of hours annually), and duration (month, day and year of commencement and termination) of each period of such employment, and the name and address of each individual, organization or entity with whom or which you were so employed. If the physical location of your practice is not within a jurisdiction where you are licensed, please be prepared to provide legal justification that demonstrates your practice was permitted in the jurisdiction where you are licensed as well as the jurisdiction where your practice occurred.
